José Alvarado, the Phillies’ most consistent reliever to begin 2025 and one of the hardest-throwing lefties in the sport, was suspended 80 games Sunday by Major League Baseball after testing positive for a performance-enhancing drug.
The league said Alvarado tested for exogenous Testosterone, a banned substance. His suspension is without pay and effective immediately. If the Phillies reach the postseason, Alvarado will be ineligible to participate.

“The Phillies fully support Major League Baseball’s Joint Prevention and Treatment Program,” the team said in a statement, “and are disappointed to hear today’s news of Jose’s violation.”
Alvarado, 29, had a 2.70 ERA with 25 strikeouts and four walks in 20 innings this season. He is in the final year of a three-year, $17.5 million deal. The Phillies hold a $9 million club option for 2026.
He came to spring training in better shape and had seen strong results from what he said was a healthier offseason.
Now, the Phillies are faced with a prolonged absence in a bullpen that has looked like a weakness for much of the season’s first seven weeks.
